When using insurance, you will pay for your visit upfront. You will then present your receipt of care to your insurance provider who will then reimburse you based on your deductible and co-pay.
Elder Athlete Physical Therapy is an original Medicare participating provider. You will pay only your co-insurance/copay as appropriate and we will submit paperwork on your behalf. Medicare will forward automatically to your secondary payer.
We are out of network for all other insurance plans.

If you have a Medicare Advantage Plan (Medicare Replacement) with Out of Network benefits, then you will be billed the allowed Medicare rate at the time of service. You will be provided a receipt/voucher and submit for reimbursement directly with your insurance carrier.
